    Acute Care at Home for Older People – An Innovative Approach to Care

    Eamon Farrell is the Manager of Acute Care at Home and Ambulatory Older Persons Services in the Southern Health and Social Care Trust.  Eamon joined the Trust in 2001 as a Physiotherapist and worked primarily in older people’s services in both Acute and Community sectors.   He became a locality manager of Intermediate Care and Community Stroke Services in 2010 prior to taking up the role of Transforming Your Care Service Implementation Lead in January 2014.  In this post Eamon led the development and implementation of the new Acute Care at Home Service for Older People and is currently managing this new service which became operational in September 2014. He is also leading a review of Day Hospital Services for Older People in the Southern Trust. Eamon is currently undertaking an MSc in Developing Practice in Healthcare through the University of Ulster which he hopes to complete in 2017.
